About Us Blue Ridge Care Connection

The Care Connection for Children Program provides community-based services and funds to support families with children with special healthcare needs.

We gather case managers, medical providers, educators and family members together to plan and manage a plan of care specific to each child. The program also provides a pool of funds to help meet the needs of uninsured and underinsured children.

Six regional programs operate throughout the commonwealth. Each program is managed locally, maximizing the program's ability to make efficient and effective use of the area's unique resources.

Our Funding Sources

Federal funding derives from the Maternal and Child Health (MCH) Services Block Grant (Title V of the Social Security Act), which ensures the health of mothers and children nationwide.

The Virginia Department of Health provides a $3 match for every $4 of federal funding.
