Accessing your child's MyChart account can help you with filling prescriptions, scheduling appointments, and seeing test results. You can learn more about how to use MyChart here.
To access your child's MyChart account, you will need to fill out proxy forms. At 13, your child's MyChart changes, and you'll need to fill out an additional form to continue to have access.
Once your child is a legal adult, you can still have caregiver access if needed.
- Parent/ Legal Guardian Proxy Access to MyChart (for a child under 13 years old)
- Parent Access for a child 13-17 years old
- Caregiver MyChart Access for Patients Over 18
