Highlights
Patient Information
You can call the clinic beginning at 8 a.m. Monday - Saturday to schedule an appointment. We offer extended evening hours on Monday, Tuesday & Thursday for sick and well visit appointments.
New Patients
Call us to make an appointment or transfer your care. Bring medical records and immunization records to the first appointment, if possible. Our front desk staff can help you with transferring records.
Forms
Give us 48-72 hours to complete form requests.
- Sports forms (VHSL) require a physical after May 1 of the current year.
- Kindergarten school entrance forms require a physical exam within one year of school start date.
- Other forms usually require a physical exam within the past one to two years.
