Being the parent of a child with ongoing medical issues requires organization and willpower. You need to make the appointments. Fill the medications. And keep everyone going when they start to feel overwhelmed. But after a few years, you start to get the hang of it. And just when you think you’ve got the routine down, your child’s growing up. And you need to hand over the reins of their medical care.

This transitional period can be stressful for your child and for you. That’s why we’ve tried to make the process as easy as possible.

Finding the Right Provider

For many, changing providers with a chronic illness is scary. If you’ve found a treatment plan that works, you worry a change in providers may disrupt that plan. If you’re still in the process, you worry you’ll have to start over from the beginning.

But transitioning care doesn’t have to be that way. When doctors communicate and navigate a planned transition, it’s easier to preserve a patient’s progress.
