Resources for Families

pediatric patient transitioning to young adult

Helping a child transition into adulthood is as important as it is challenging. As they take charge of their health, you may have questions. These resources can help.

Access & Medical Records

When your child is very young, being able to access their MyChart account can help you keep track of visits. But you'll need a special form to access their health records. As they get older, you'll have different access options. For any access though, you'll need to fill out information forms.

These are the forms you'll need as a caregiver

Preparing for Visits

The Question Builder tool helps you and your child prepare their list of questions before a doctor's visit.

Supporting One on One Time with a Healthcare Provider: CDC helps you know how to encourage your child to talk to their doctor.
